COVENANT-PROVISION-INDEPENDENT OF REPRESENTATIONS
[Covenantor] understands that the personal injury sustained as a result of [description of accident] are or may be permanent and progressive and that recovery therefrom is uncertain and indefinite. In making this covenant, [covenantor] relies wholly on [his or her] own judgment, belief and knowledge of the nature, extent and duration of such injuries. No representations or statements regarding these injuries or any other matter made by [covenantee] or by [his or her] agents or representatives, or by any physician or surgeon employed by [him or her] have been relied on to any extent whatever in making this covenant.
